Pretty sure it's real. He actually passed away in 2005 when he was found unconscious in a hotel room from acute cardiac failure. He had an "underlying atherosclerosis cardiovascular disease."
Reports say it was real because a couple of nights before he fought JBL in a serious match and was surprisingly opened up meaning he lost a lot of red fluid from physique.
